PHMSA Advisory Bulletin: Mechanical Fitting Failure Reports (June 11, 2012) 

On June 11, 2012 PHMSA published an advisory bulletin relating to mechanical fitting failure reporting to the Federal Register.  After reviewing the mechanical fitting failure reports that were submitted for 2011, PHMSA determined that operators needed additional quidance for reporting apparent cause.  This advisory bulletin clarifies reporting requirements for the Mechanical Fitting Failure Report Form, PHMSA F 7100.1–2.
