2012 PHMSA Gas Transmission Annual Report Summary

A summary of the 2012 PHMSA Gas Transmission Annual Report data is now available for download. The summary delineates national operator transmission mileage that falls under CFR 192.619 for MAOP verification and record completeness, grandfathering, and pressure tested pipe separated by class and HCA. Also outlined is mileage that falls under NTSB recommendations P-11-14, P-11-15 and P-11-17 for grandfathering, manufacturing/construction defects, and ILI-able pipeline.

Advisory Bulletin - Pipeline Safety - Using Meaningful Metrics in Conducting Integrity Management Program Evaluations

PHMSA issued an Advisory Bulletin to remind operators of gas transmission and hazardous liquid pipeline facilities of their responsibilities, under Federal integrity management regulations, to perform evaluations of their integrity management programs using meaningful performance metrics.

Final NTSB Accident Report: San Bruno Pipeline Rupture (Sept. 26, 2011)

On September 26, 2011 the National Transportation Safety Board (NTSB) made public the accident investigation report "Pacific Gas and Electric Company Natural Gas Transmission Pipeline Rupture and Fire, San Bruno, California, September 9, 2010"(NTSB/PAR-11/01, PB2011-916501). This report includes NTSB's findings regarding the facts involved in the accident, the cause of the pipeline rupture, an assessment of the PG&E's actions and recommendations that were issued as a result of this accident investigation.
PHMSA Advisory Bulletin: Cast Iron Pipe (March 23, 2012)

On March 23, 2012 PHMSA published an advisory bulletin in the Federal Register to reiterate the cast iron recommendations included in two 1991 and 1992 Alert Notices and address cast iron pipeline requirements specified in the Pipeline Safety, Regulatory Certainty, and Job Creation Act of 2011.
PHMSA Advisory Bulletin: Clarifications Regarding the Implementation of the National Registry for Operators (January 13, 2012)

On January 13, 2012 PHMSA issued an Advisory Bulletin to notify operators of pipeline facilities of PHMSA s plan regarding theimplementation of the national registry of pipeline and liquefied natural gas operators. This advisory bulletin provides a table that identifies the expected submission dates for the various submissions that are related to the national pipeline operator registry. In addition, PHMSA provided a few clarifying questions and responses.
PHMSA Advisory Bulletin: Mechanical Fitting Failure Reports (June 11, 2012)

On June 11, 2012 PHMSA published an advisory bulletin relating to mechanical fitting failure reporting to the Federal Register. After reviewing the mechanical fitting failure reports that were submitted for 2011, PHMSA determined that operators needed additional quidance for reporting apparent cause. This advisory bulletin clarifies reporting requirements for the Mechanical Fitting Failure Report Form, PHMSA F 7100.1 2.
PHMSA Advisory Bulletin: Validation & Registry Requirement Changes for the National Registry for Operators (March 21, 2012)

On March 21, 2012 PHMSA issued an Advisory Bulletin to notify operators of changes that are being applied to the National Registry of Pipeline and Liquefied Natural Gas Operators.
PHMSA Advisory Bulletin: Verification of Records (May 7, 2012)

On May 7, 2012 PHMSA issued an Advisory Bulletin (AB) to remind operators of gas and hazardous liquid pipeline facilities to verify their records relating to operating specifications for maximum allowable operating pressure (MAOP) required by 49 CFR 192.517 Records and maximum operating pressure (MOP) required by 49 CFR 195.310 Records. This AB provided guidance and definitions for the terms traceable, verifiable and complete. PHMSA provided guidance and examples to clarify what constitutes as sufficient supplemental support to validate a pipeline's MAOP.

PHMSA Notice and Request for Comment: Changes to Transmission and Gathering Pipeline Systems Annual Reporting (April 13, 2012)

On April 13, 2012 PHMSA published a notice in the Federal Register advising they will request approval from the Office of Management and Budget (OMB) for the revision of several forms in two currently approved information collections. They are considering a number of revisions to the annual report form for gas transmission and gathering pipeline systems.
PHMSA Notice and Request for Comment: Excess Flow Valve Information Collection (May 15, 2012)

On May 15, 2012 PHMSA published a notice to the Federal Register to address NTSB's Safety Recommendation P 01 2 and the EFV requirements mandated by Congress in the Pipeline Safety, Regulatory Certainty, and Job Creation Act of 2011. NTSB's Safety Recommendation P 01 2 advised DOT to require the expanded use of EFVs in gas distribution systems, when available technology can be applied.
PHMSA Notice of Proposed Rulemaking: Pipeline Damage Prevention Programs (April 2, 2012)

On April 2, 2012 PHMSA published a Notice of Proposed Rulemaking to the Federal Register to revise Pipeline Safety Regulations. This notice proposes criteria, procedures and an administrative process for determining the adequacy of state pipeline excavation damage prevention law enforcement programs. Additionally, this notice proposes Federal requirements that PHMSA will enforce in states with inadequate excavation damage prevention law enforcement programs and proposes an adjudication process for administrative enforcement proceedings against excavators where Federal authority is
PHMSA Guidance for Pipeline Safety Evaluation & Metrics (Oct 15, 2014)

On October 15, PHMSA published in the Federal Register an Advisory Bulletin informing operators of “elements and characteristics of a mature Integrity Management program evaluation process using meaningful metrics” through a document titled “Guidance for Strengthening Pipeline Safety Through Rigorous Program Evaluation and Meaningful Metrics.” (also attached).PHMSA states that the guidance document:… should be used when operators develop and perform IM program evaluations.
